PAPERWORK HAND-IN & EQUIPMENT HAND-OUT

Wednesday - July 12 & July 19
6:30 to 7:30
at the equipment trailer
Located behind the Carver Police Station to the left of the baseball fields.
YOU MUST have all paperwork handed in to get equipment
Please remember to bring:
A copy of your child’s completed report card for the 2005/2006 academic year, this copy must show grades in ALL terms of the academic year.
A medical release signed by child’s doctor in the calendar year 2006
For all children that did not participate last year, we must have a birth certificate with a raised seal from the town. This can not be the hospital copy and can not be a photo copy. All original birth certificates will be given back in September by the team mother.

This will be the final opportunity to sign up with a registration fee of $100 per child. The registration will increase to $125 on August 1.
If you plan to sign up on either of these nights, please bring all of the paper work we need for you child with you.

***FIRST NIGHT OF PRACTICE***

Wednesday - August 2nd
6:00 to 8:00 PM
In the Auditorium at the Carver High School

This is a mandatory meeting for all participants and a Parent or Guardian. New information regarding the 2006 season will be reviewed and the children will get their team assignments. If for any reason you can not make this meeting you must notify us at 508-453-1211 so arrangements can be made to make sure you have all the information you need for you child to start the season. If you plan to register in August, please come to the meeting but NO registrations will be accepted on this night. We will provide instructions for late registration at that time.

PAPERWORK NOTE: ALL paperwork needs submitted before a child can begin to practice, there are NO exceptions! This is an insurance requirement for all participants. The board will not jeopardize the insurance coverage for the entire program for anyone so again there are NO exceptions!
